This form contains information relating to the disclosure of your personal Data and Sensitive Personal Data, as defined by the Data Protection Act 1998 (the ‘’Act’’) to third parties and Clients. As such, please read this document thoroughly before signing below.

By ticking this box I confirm that the information provided on my application form and within my CV is both truthful and accurate. I have omitted no facts that could affect my future employment and that any engagement entered into is subject to documentary evidence of my right to work in the UK, verification of any professional qualifications and satisfactory references, medical assessments and criminal records checks. Any misleading statement or deliberate omission may disqualify my application and lead to termination of my assignment. The neutral vendor, supplier or client may themselves verify any of these documents at any time. In accordance with neutral vendor, suppliers and Clients for Safeguarding vulnerable adults and children. If you are placed into a role where you will be working unsupervised with vulnerable persons, we will also be required to disclose a copy of your Disclosure and Barring Service (DBS) Check. We will scan both the Certificate, a copy of your photo I.D. and where applicable your wok visa and upload the scans to the neutral vendor Supplier’s or Clients I.T system. I hereby confirm that I have read and understood the information detailed above and that I give my express consent (a) for you to disclose the entire contents of my personnel file to employees or agents of neutral vendor, Suppliers or the Client; (b) for neutral vendor, Supplier or the client to verify any of the details disclosed; and (c) for you to scan and upload a copy of my DBS disclosure Certificate, Photo I.D and where applicable my work visa to neutral vendors, suppliers and Clients.
